Sharon Chamberlain, patient partner volunteer on Fraser Health’s Patient Advisory Council, shares how she is living Our Shared Commitments:

“Our mother was 82 and lived independently in her Burnaby condominium. She loved knitting and reading -- she especially enjoyed family get-togethers and spoiling her grandchildren. She had health problems and saw her primary care providers to manage her diabetes, COPD, and liver and kidney disease. One day mom fell and broke her hip. To recover she would require surgery and rehabilitation.

Subsequently, our mother experienced what would become a 15-month health care journey in five Fraser Health care sites. Keeping her care providers informed throughout this process was our family’s top priority to ensure the best care for our mother.

We value honest and open dialogue. We made the commitment to share all of our mother’s health information with her care teams to ensure everyone had a full understanding of her health history upon which to establish an effective care plan. It is the responsibility of the patient and their family to be involved in care planning to ensure quality care. It’s also important that patients and families know who to talk to when they have questions or want to discuss matters that may affect the patient’s care goals.

I am a volunteer on Fraser Health’s Patient Advisory Council. I ensure the patient voice is heard at the table where health care policy decisions are made. I have enjoyed working with Professional Practice to assist them in designing patient-centred training for patient care coordinators. I have also had the opportunity to partner with other patient advisors and the Patient Experience Team to create Our Shared Commitments. It is our hope that these commitments will further foster positive care experiences for patients and families.”
